This program is designed for our patients and families who refer friends to Due West Orthodontics. While we love referrals from everyone, rewards are reserved for patient referrals only.

Getting started is easy:

Grab Referral Cards

Stop by our office and pick up referral cards. Fill in your:

      • First & last name
      • Phone number
      • Email

Then pass them out anywhere — school, sports, church, neighborhood… wherever your people are.

Share Online

Send your friends directly to:

They can click, print, and bring in the referral card.

⚠️ Important: Your friend must complete the referral information so we can properly credit you.

  • Rewards are issued only when a referred patient begins orthodontic treatment.
  • This program is intended for individual patient referrals only.
  • Dental professionals and employees of dental or orthodontic offices are not eligible to participate in this referral program.
  • Participation may be limited where prohibited by applicable laws or professional/ethical guidelines.
